Purpose of form:
U.S. payors file this form for each person to whom they:
- Paid dividends (including capital gain dividends and exempt-interest dividends) and other distributions on stock of $10 or more,
- Withheld and paid any non-U.S. tax on dividends and other distributions on stock,
- Withheld any U.S. federal income tax on dividends under the backup withholding rules, or
- Paid $600 or more as part of a liquidation.
Information reported on form:
- The name, address, taxpayer identification number and phone number of the payor,
- The name, address and taxpayer identification number of the recipient,
- The aggregate amount of payments of dividends (including capital gain dividends and exempt-interest dividends) and other distributions on stock of $10 or more (including the type of dividend),
- Amounts of non-U.S. tax withheld and paid on dividends and other distributions on stock and the foreign country.
- Federal income tax withheld, or
- Payments of $600 or more (cash and non-cash payments) as part of a liquidation.
